1. MD or DO
2. Active AZ medical license
3. Active DEA
4. Must be DOT certified

What you need to know:
– 7:30 am – 4:30 pm, Monday – Friday
– Active AZ license
– Board Certified or Board Eligible
– 1+ years of experience
– Travel costs covered
– Malpractice insurance covered
