Job Summary:

The Home Heal – Physical Therapist will be responsible for providing physical therapy services to patients in their homes. The Physical Therapist will work with patients to develop treatment plans, monitor progress, and adjust treatment as needed. 7. Instructs patient, significant others, and staff in the care
and use of wheelchairs, prosthetic, and orthopedic
devices; and in areas of physical therapy in which they can
participate and assist the patient.
8. Visits patient according to Plan of Treatment; completes a
progress note for each visit; and submits progress notes
to the agency on an at least weekly basis.
8. Participates in staff conferences and committees as
necessary.
9. Attends all education and in services as required.
10. Prepares a written discharge summary and a written
summary report on patient’s condition at least every 60
days.
