We are looking for people interested in joining an onsite session in Downtown Los Angeles, California. The RN Care Coordinator works within a multi-disciplinary health care team in the delivery of comprehensive services for patients who are identified as high utilizers of the ED or Hospital and/or diagnosed with chronic diseases such as, but not limited, to diabetes, hypertension, obesity, chronic pain, or Hepatitis C. The RN Care Coordinator is a patient advocate and facilitator of the Patient Centered Medical Home (PCMH) and its target population of Diabetics and Emergency Room / Hospital Utilizers through the provision of health education, medication adherence counseling, follow-up monitoring, improvement of access to care, and guiding patients through problem focused clinical visits.
Qualifications and abilities
● Registered Nurse in the State of Hawaii. RN License is current, valid & in good standing
● Familiarity with common software programs, (i.e. Windows-based programs) an ability to quickly learn new systems.
● Ability to communicate effectively with patients, community members, and staff members.
● Proficient in computer skills: typing, navigating through the internet and emails.
● OSHA training.
● BCLS biannually.
General Duties include but are not limited to
● Collaborates with medical providers and staff members to identify patients via referral from a target PCMH population who clinically demonstrate (i.e. high ER utilizer, uncontrolled A1c) need of care coordination.
● Assists health care team and patient to develop person-centered care plans tailored to patient centered needs and primary social support system.
● Assesses patients for care coordination by asking and receiving consent to have care coordination, assessing disease and treatment comprehension, depression, labs, medications, and patientÂ’s health related goals.
● Complete ER/Hospital follow-up calls in a timely manner. The goal is to get the patient in to the clinic to be seen by a provider within 7 days of discharged
● Assess patientÂ’s social determinants of health (SDOH) and provide information on community resources.
● Other duties as assigned.

Position Summary
McMorran House is the official residence of the university President. Under the direction of the Administrative and Operations Manager for the Office of the President and McMorran House, the McMorran House Coordinator and Special Events Manager is responsible for coordinating and assisting with McMorran House Operations as well as the planning, coordination, implementation, and evaluation of special events, including but not limited to events for the McMorran House, and select other University events.

The Coordinator will be responsible for implementing the strategic plan developed by the Office of the President to engage the universityÂ’s diverse internal and external audiences including the greater community, alumni, friends, faculty, staff, volunteers, and donors. In addition, this position will maintain a systematic goal-setting process and a formal evaluation procedure for McMorran events.

The Coordinator serves as the principal contact on all matters relating to the annual, monthly, and daily maintenance and upkeep of the president’s residence, overseeing maintenance and ensuring that the public spaces of the residence are in a state of complete readiness at all times.

This position requires a flexible work schedule, working on campus and on-site at McMorran House to include some evenings and weekends. May also require on-call work on nights and weekends as point of contact for McMorran House facilities issues.

Position requires knowledge of and demonstrated experience working and communicating with senior leadership, administrators and donors and the ability to Interact with all levels of campus constituents (including president and vice presidents) and other high-level VIPs (trustees, donors, etc.) to create and maintain cordial working relationships to ensure success of events and house operations.

This position demonstrates strong communication, interpersonal, planning and problem-solving skills, the ability to respond to needs as they arise, and conducts all job duties with high levels of confidentiality, sensitivity, and cultural awareness.

This position requires the ability to travel to the McMorran house, and to run errands, sometimes on short notice.

Driving Requirement:
It is an essential requirement of this position to qualify for and maintain UO driver’s certification eligibility, which includes maintaining an Oregon driverÂ’s license throughout employment in this position.
