Family Practice Physician – Locum

Required Skills & Experience:
1. Licensed Physician in Illinois
2. DEA
3. 1+ years of experience
4. ABFM or AOA Board Certified MD/DO in Family Medicine OR Truly BE (Less that 2 years out of training)
5. BLS

What you need to know:
– No Internal Medicine Physicians as they’ll need to see all ages
– Ongoing locum assignment starting in January.
– Schedule: Locum need is M-F ongoing or an extended period of time starting in January 2020. Due to continuity of care, we are looking for FTE locum providers that can do a minimum of 3 weeks per month.
o Clinic hours of operation are Mon – Wed 7:30a-8p and Th & Fr 7:30a – 5p.
o The locum may/may not be required to work during all open clinic hours and up for discussion.
