We offer you a full time opportunity in Cypress, with travel to Tomball as needed

What we want to see from you:

A graduate of an approved Registered Nursing educational program.
Active and current Texas RN registration in good standing.
Must be able to read, write and speak English.
Background in home health, community nursing, geriatrics, behavioral health, psychiatric nursing, and/or in the field of intellectual and/or developmental disabilities prefered.
Must have a valid driver’s license and a clear driving record.
Must have cleared and approved criminal record check.
CPR certification or willingness to obtain.
Must have ability to travel among assigned sites.
Able to do frequent bending and lifting; push and pull 25 pounds.
Frequent walking, bending, and stooping.

Knowledge and experience with electronic medical records and other computer operations.
If not currently certified, willingness and ability to complete Bethesda Lutheran Communities CPR course.
Ability to provide essential training courses for Bethesda staff as mandated and directed by each region.
Respect for the individuals supported and commitment to assist them in achieving their best possible health.
Ability to work independently as well as collaboratively as needed.
What you will be doing:

Establishes a therapeutic relationship with the person and maintains that relationship within boundaries.
Collaborates as part of the interdisciplinary team in developing, implementing, and evaluating the plan.
Collects health and functional status data for persons with intellectual and/or developmental disabilities systematically and continuously.
Ensures the data are documented, communicated, and made readily accessible.
Identifies health care needs for the person based on the data collected through the nursing process.
Develops a plan with goals and interventions specific to the needs of the person.
Implements the plan to promote, maintain, or restore wellness, prevent illness, and/or provide appropriate end-of-life care.
Evaluates response of the person to the interventions and supports provided and revises the plan accordingly.
Participates in the quality assurance process in order to analyze and assure continuous improvement in the quality of health care supports.
Supports individuals to achieve their best possible health and to manage their own health care.
Responsible for educating the individuals under his/her care, family members, members of the individual’s support team and the public, including health care providers; including information about the individual’s care and needs, as well as about the health care needs of an individual with intellectual and/or developmental disability in general.
Responsible for instructing unlicensed personnel in health related subjects and courses as required by the program licensure where individuals are supported.
Assumes responsibility for continued competence, professional development and accountability.
Obtains sufficient CEUs for continued active RN licensure in the state of practice.
Provides nursing care according to the physician’s orders, in compliance with recognized nursing standards and State and Federal Regulations.
Participates in quality assurance data collection and reporting as indicated by program requirements.
Provides supervision and direction of health provided by LPNs and/or unlicensed personnel.
