Requirements:
1. Fill out an application form
2. A copy of your DBA (Doing Business As)
3. A copy of your EIN (Employer Identification Number)
The DBA is the registration of your business with the county clerk of your city of residency.
I believe hey charge between $25-$30 to register your business and the registration is valid for 10 years.
The EIN is a unique identification number that is assigned to your business so that it can easily be identified by the IRS.

There is no charge to obtain an EIN when you apply directly with the IRS. If you decide to ask the person/company that does your income tax, they might charge you.
