Home Health RN that manages cases from startups to re-certification, to discharge. Must have at least 2 years of nursing with acute care, and 1-2 years home care with previous Oasis experience; prefers Healthwyse Documentation as well. Must be independent, comfortable initiating calls to Dr.’s, pharmacists, specialists, etc. and able to cover assigned geographic location with a 6 point productivity per day. These are acute patients that may have chest tubes/ pleurex, Foley catheters, wound care and wound vacs, labs, cardiac assessments. Acute care home health
