Job responsibilities

RN Care Manager

The Registered Nurse Care Manager’s primary focus is assisting providers and patients in the areas of nursing care management. The RN Care Manager will assist the Clinic Nurse Manager as needed and serves as a member of the Care Team. The RN Care Manager reports directly to the Clinic Manager.

Specific duties and responsibilities:

Work as part of the clinic nursing team to provide a full range of nursing services including floor nursing, patient triage, phone advice, case management, and patient education.
Work closely with the Primary Care Provider(s) and clinical Care Team in providing Nurse Care Management which may include:
Coordination of care within primary Care Team(s)
Proactive management of patients with chronic illness with poor clinical outcomes or with complex healthcare/social needs
Assist patients in care management and system navigation with recent critical diagnosis
Assist Providers with management of critical abnormal lab/imaging results
Assist with the care transition for patients discharged from an inpatient setting
Active participation in Care Team’s “High Risk Case Management” program
Provide home visits as indicated
Assist with set up of flu/COVID vaccination clinics, oversight of inventory, appointment scheduling, redistribution of vaccines, staying current on flu/Covid vaccination guidelines
Work with patients and Providers to assess, identify problems, plan goals, monitor and evaluate client care plans and develop strategies that meet the client’s immediate and long-term needs.
Work closely with Primary Care Provider(s) to manage patient care needs which could include: medication refills per protocol, lab triage, document management, management and ordering of durable medical equipment, prior authorizations for needed medications or other services, and other items or medical and social services as needed.
Assist with population management of patient panels.
Assist patients in obtaining appropriate medications and other medical supplies and equipment.
Participate in patient and staff education at the direction of the Clinic Manager.
Provide Nurse visits for designated conditions per clinical protocol/Provider request.
Coordinate Care Team support for patients with new health management goal.
Coordinate tracking of patients for follow-up and continuity of care which may include:
Developing and using practice guidelines and protocols;
Use of flow sheets, problem lists, and medical data bases;
Assist with discharge planning / home care referrals.
Maintains patient confidentiality at all times; abides by HIPPA and other regulations put forth by the various regulatory agencies.
In the absence of the Clinic Manager (e.g. illness or vacation), the Nurse Care Manager will be able to step in as acting Clinic Manager on a temporary basis.
Performs other duties as assigned by the Clinic Manager.

Job Summary

What you need to have in your toolbox:

A 4 Year / bachelor’s degree from an accredited school of nursing is required.

AWR Reports.

Should have a clear understanding of Oracle Server Process Architecture and
memory structures PGA (Program Global Areas) & SGA (System Global Areas) to
optimize memory usage.

Implement ETL solutions for large volume of data by applying various data
warehouse modeling techniques like Star Schema, Data partitioning, Data
compression, Direct path loads etc. and that maximizes re-usable components

and services that incorporate versioning, reconciliation and exception
handling

Experience in Unix/Linux shell scripting and/or Perl Scripting. Knowledge of
python is desirable.

Implementation experience with monitoring and measurement of data quality
using business rules. Hands on experience hands with data quality assessment
and resolution desirable.

Maintain metadata, data glossary and data catalog processes. Ensure adherence
to data security by using data encryption procedures and protocols.

Ability to work in fast paced environment

Experience in Financial Industry with some risk background.

Knowledge of Agile methodology is desirable
